While many patients are self-referred, we encourage all to have a primary provider with whom we coordinate care. If you don't have one, we would be happy to proved a referral. Pertinent past records, such as blood work, imaging or other test reports, might be helpful at the time of your initial evaluation. We provide the full range of inpatient & outpatient services from testing only at the request of other providers, to long term specialty care for chronic conditions.
